Please review the following conditions before submitting an article reproduction request.

Conditions

1. Content may not be modified, except style or format changes
2. No trademark, logos, or advertisements are permitted to be reproduced with the copyrighted materials.
3. No disclaimers are permitted to be reproduced within the copyrighted materials, but can be placed on a coversheet.
4. Permission is granted for an English language, one-time use only.
5. Permission must be requested separately for future editions, revisions, translations, derivative works, and promotional pieces.
6. Permission for online publication will not be given; permission to link to the original article can be requested.

Article reproduction requests should include the following:

1. Correct contact information for the requestor, including name, organization, department, address, phone, fax and email.
2. Correct and complete citation information for the requested material, including journal title, author, article title, year, and volume.
3. Where the material is to be reproduced and how many copies will be printed.

Pricing is determined based on number of pages in an article, and amount of copies requested. 

Sample Rates for 12 pages:

500 copies:  $2,110.00 USD
2,000 copies: $4,633.00 USD
5,000 copies: $9,104.00 USD

If you decide to proceed with your reproduction request, an invoice and Statement of Permission will be created.  The Statement of Permission will need to be signed and returned, and the invoice paid and processed before permission to proceed with the reproduction can be granted.
